Our location is on the west side of France Avenue at 6600 France Avenue suite 635. Appointments may be made in our Edina Office on Tuesday and Thursday afternoons. Because of new insurance and government regulations, we are required to obtain more information from each patient than in the past, but have tried to keep our forms straightforward and easy to understand. These forms are available on this website Home Page under “PATIENT FORMS” and should be printed and completed and brought to our office at the time of your visit. Most prescriptions, except pain medication, are now being escribed to your preferred pharmacy. Please have that information available.
